Since the implementation of the Health & Social Care Act 2012 we continue to adapt to the evolving organisational and service environment, and are constantly looking for opportunities to innovate. Key to this is the creation of strategic partnerships with commissioners, who determine the way forward for their local populations.

With a significant track record of working with organisations across the health and social care system, Community Ventures is aligned with the NHS’s organisations.

Clinical Commissioning Groups (CCGs) are now at the forefront of ensuring that care is integrated and delivered in the community, with valuable input from local people and patients, providing patients with better, seamless and more accessible care. This is a key role to see services transferred from acute to community settings in line with the Five Year Forward View. We are working with CCGs across Yorkshire and the North East including: Leeds North; Leeds South and East; Leeds West; Calderdale; Wakefield; Hartlepool and Stockton on Tees; Durham Dales; Easington and Sedgefield; Darlington; North Durham; South Tees; Hambleton, Richmondshire and Whitby; Scarborough and Ryedale, Harrogate and Rural; Vale of York.

We are also working closely with the DH, NHS England area teams and those playing a pivotal role in local strategic estate planning.

Other commissioners with whom we are working include Community Health Partnerships and NHS Property Services in Yorkshire as specialist estates organisations supporting front line clinical care.

Whitby Hospital Strategic Estate Options Appraisal 2014

Community Ventures was commissioned to undertake this wide-ranging strategic review in response to the Hambleton, Richmondshire and Whitby CCG’s Strategic Commissioning Strategy. The CCG wanted to deliver integrated services closer to people’s homes by developing a health and wellbeing hub, community urgent care centre, and a redesign of inpatient and outpatient services.
